Obat selektif perifer

Obat selektif perifer adalah obat yang memiliki mekanisme kerja utama di luar sistem saraf pusat (SSP), biasanya karena terhalang masuk ke SSP oleh sawar darah otak. Dengan terhalang masuk ke SSP, obat-obatan dapat bekerja pada bagian tubuh lainnya tanpa menimbulkan efek samping yang berkaitan dengan pengaruhnya pada otak atau sumsum tulang belakang. Misalnya, sebagian besar opioid menyebabkan sedasi jika diberikan dalam dosis yang cukup tinggi, tetapi opioid selektif perifer dapat bekerja pada bagian tubuh lainnya tanpa memasuki otak dan cenderung tidak menyebabkan sedasi.[1] Opioid selektif perifer ini dapat digunakan sebagai antidiare, misalnya loperamida.[2]

Mekanisme selektivitas perifer meliputi hidrofilisitas fisikokimia dan ukuran molekul yang besar, yang mencegah permeasi obat melalui membran sel dwilapis lipid sawar darah otak, dan pengeluaran obat dari otak oleh transporter sawar darah otak seperti P-glikoprotein di antara banyak lainnya.[2][3][4] Transportasi keluar dari otak oleh P-glikoprotein dianggap bertanggung jawab atas selektivitas perifer banyak obat termasuk loperamida, domperidon, feksofenadin, bilastin, setirizin, ivermektin, dan deksametason.[2][5][6][7][8]
